Job Description

Exciting opportunity for a full-time, experienced, hospital employed, Advanced Practice Provider to be part of a new Rural Health Clinic. Advanced Practice Provider will work through a collaborative agreement with a Family Medicine Physician to provide care and services to patients from pediatrics through geriatrics.
Advanced Practice Provider would need at least three years of experience in primary care, and be licensed to practice in the State of Illinois.
This provider may provide cross coverage in other hospital clinics if deemed necessary.

Typical call schedule would be 1:2 with rotating weekends. Provider will round on inpatients and be available via phone.

Perry Memorial Hospital has over 100 physicians on staff and state-of-the-art medical equipment. The hospital is designated as a Critical Access Hospital which allows the hospital to receive better reimbursement for Medicare patients, assuring its financial stability. The hospital offers a 24-hour physician staffed Emergency Department that is equipped with state of the art trauma rooms seeing approximately 10,000 patients per year. Perry Memorial also provides intensive care, adult, geriatric and pediatric medical care, and a birthing center. Inpatient and outpatient surgery and rehabilitation services, pain management, vascular and GI procedures. Homecare and hospice services are all available. All digital diagnostic services including 1.5 MRI, 64 slice CT, X-ray, mammography, 3D ultrasound, angiography, EKG, and sleep studies. The desire to work in a collegial environment is important.
