Job Description
Modern CHC that serves 50,000 low income patients in the Rockford/Northwest Illinois Region. We are expanding from five to six very attractive primary care sites. We are looking for Family Physicians to join our team of 80BE/BC provider team. On-site includes integrated Behavioral Health, PCMH, Medical Pain Management and other specialty services.
