Job Description

Go Docs Go, a physician house call practice, is seeking both full-time and part-time Nurse Practitioners/Physician Assistants as primary care providers to provide at home care to our geriatric and disabled patient base, primarily Medicare recipients, residing in the counties listed below. We have an immediate need for someone in the Lansing and Hillsdale areas. The Go Docs Go practice uses a physician / nurse practitioner team model and provides exceptional care by bringing compassionate providers directly to your door.
SUMMARY:
The NP/PA will conduct primary care patient visits in private homes, group homes, and independent or assisted living facilities. Patients are visited approximately every four to six weeks in an effort to promote health maintenance and avoid unnecessary hospital visits. The practice will emphasize health promotion and maintenance, disease prevention, and the diagnosis and treatment of acute and chronic health problems. The Nurse Practitioner will work collaboratively with the physicians, hospitals, skilled nursing facilities, patients residential facility, as well as clients in their own home to develop care protocols.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Conducts medical evaluations of predominantly elderly, frail, chronically ill homebound patients in their home environment.
Documents visit completely and accurately via EMR at the time of the visit for medical record and billing purposes.
Develops a treatment plan in collaboration with the physician, patient, and significant others
Evaluates patient medication and writes/orders prescriptions as needed.
Performs minor surgical procedures when required.
Refers patients to physicians as appropriate for consultation or for specialized health resources and treatment.
Maintains current knowledge of and promotes compliance with all federal, state, and local regulations. Keeps fully cognizant of current Medicare, Medicaid, and third-party reimbursement regulations and changes and ensures compliance.
Maintains patient and staff privacy and confidentiality at all times pursuant to HIPAA Privacy Final Rule.
Meets minimum productivity requirements based on practice standard.
IDEAL CANDIDATE WILL HAVE:
Minimum education: Masters required
Adult, Geriatric, Family or Acute Care specialties preferred
1 year experience as NP/PA preferred; Home Care nursing experience a plus
Active, unencumbered nursing license
Nurse Practitioner certification from either the ANCC or AANP, a plus
Must be able to work independently in the field, driving and navigation skills (full-time NP/PA are provided w/ company car and M.A. if desired)
Must have working knowledge of computers, cell phone, fax machines, internet
Familiarity with ICD-9 and CPT coding
Superior communication skills required
Excellent organizational skills with ability to multi-task
