SUMMARY
Stephen F. Austin Community Health Network is seeking a board certified (board eligible) Family Practice physician to work in our Freeport, Texas facility. Enjoy a full or part-time outpatient only position at a Federally Qualified Health Center (FQHC) in Brazoria County, Texas that provides professional growth and development in a group practice providing primary health care for adults and children, womens health services, behavioral health and general dentistry. Stephen F. Austin Community Health Network is dedicated to continuous quality improvement and the ideal candidate will work within a team to insure comprehensive and excellent care. SFACHN clinics are dedicated to Patient Centered Medical Home (PCMH) principles and practices.

Responsibilities include: meeting or exceeding productivity requirements as set forth by the Chief Executive Officer and the Chief Medical Officer; performing assessments and providing treatment to clients of the center; developing and maintaining clinical diagnostic and treatment protocols, providing supervision for non-physician providers and participating with committees as necessary.

BRAZORIA COUNTY is adjacent to Harris County (City of Houston), Ft. Bend County and Galveston County. Enjoy the city, the country and the Gulf in this ideal location. Our main office, located in the city of Alvin is 31 miles from downtown Houston, 15 miles from NASA and 30 miles to Galveston Island. The Freeport office is located in the Brazosport area includes the cities of Clute, Freeport, Lake Jackson and Surfside Beach. It is located on the Texas Gulf Coast and is a short drive from Houston.
