Community Memorial and Family Health Centers, located in Hamilton NY is a cornerstone of health and wellness for Madison and surrounding counties. We are a critical access hospital and in conjunction with our Health Centers we offer essential preventative, primary, behavioral and acute care services. Community Memorial is the ONLY hospital in New York State to achieve one of Healthgrades "America's 100 Best Hospitals for Joint Replacement", HCAHPS Best Performer, and our Hamilton Family Health Center is a Patient Centered Medical Home, NCQA Level III.

Join our Primary Care Outpatient Practices, our positions include the following:
7:30am- 5:00pm schedule
Average 20 patients/ day
Office call only, no Hospital call
