Physician - Jerauld County Community Health Center

For over 35 years, Horizon Health Care, Inc. has been providing personalized, affordable, high-quality medical, dental and mental health care through a rural, community-based network in South Dakota. Horizon is a Federally Qualified
Health Center (FQHC) that serves the medical, dental and mental health care needs of South Dakotans in rural, medically underserved areas through 83,000 patient visits annually in 24 medical and dental clinics across South Dakota.

Grant funding allows Horizon clinics to offer
discounted fee schedules for medical and
dental care.
Horizon offers cutting edge technology like
telemedicine, electronic health records and
distance learning.
Specialty outreach clinics include surgery,
orthopedics, OB/GYN, podiatry, psychiatry
and behavioral health.

Attached to the clinic is a 25 bed, acute care Critical Access Hospital networked with hospitals in Mitchell and Sioux Falls, SD.

Wessington Springs is a family centered, rural community with 1,000 residents and has a service area of a population of 9,000 that offers innovative educational opportunities and a wealth of extracurricular activities such as, acres of wildlife habitat and recreation waters, residents and visitors can experience outdoor adventure at its best through hunting, fishing, hiking, horseback riding, snowmobiling, cross-country skiing and water sports on the nearby Missouri River.
