Nurse Practitioner

Job Description

Job Summary:

The Nurse Practitioner is responsible for providing primary care to the patients of Bluestone Physician Services under the direction of a Physician. The Nurse Practitioner and Physician work as a team, traveling to residential communities to provide on-site primary care.

Responsibilities:

*Provide regular on-site primary care to patients in residential settings
*Develop and implement clinical plans of care utilizing age appropriate palliative care
*Medication review and reconciliation, order labs as necessary
*Discuss plan of care with facility nurse and family
*Rotate responsibility for emergency phone calls
*Meet best practice and quality requirements

Qualifications:

*Graduate of NP program from an accredited school. Must have and maintain a valid Wisconsin License
*Professional clinical experience in Family Practice, Internal Medicine, Geriatric training or interest, preferred
*Proven ability to communicate effectively with strong verbal skills
*Excellent interpersonal and customer service skills
*Must have a valid drivers license and minimum state required liability auto insurance; local travel is required
*Demonstrated compatibility with Bluestones mission and operating philosophies
*Demonstrated ability to read, write, speak, and understand the English language
