Job Description

Baystate Health, an awardwinning healthcare system and home of the University of Massachusetts Medical School-Baystate Health, is seeking a Nurse Practitioner or Physician Assistant to join East Longmeadow Adult Medicine in East Longmeadow, MA.

POSITION DETAILS:

Join our well-established team of 6 physicians and 1 advanced practitioners who provide 100% outpatient primary care to adults and children in our state-of-the-art level three patient centered medical home.

Provide team-based care with exceptional support staff. 1:1 Medical Assistant and Nursing support.

Telehealth Medicine and direct access to specialists, care management, integrated behavioral health as well as, strong support from Baystate Medical Center / University of Massachusetts Medical School-Baystate Health.

The ideal candidate will be BC/BE Nurse Practitioner or Physician Assistant. Candidates must have excellent communication and interpersonal skills, strong computer skills, and have a commitment to patient-centered care.
