Job Description
This well-regarded Wisconsin-based hospital system seeks to add Advanced Practice Providers for its Geriatrics program. There are multiple positions available in locations across the state, with the overall goal being to increase the APP to MD ratio across the service.
Here are a few details:
Will treat patients in the Nursing Home setting, to improve access and reduce hospital readmissions.
Monday-Friday, 8a-5p schedule.
Hospital-employed position with full benefits.
Will receive mileage reimbursement and company cell phone.
Will work with physicians and support staff to create and carry out care plans.
