AMM provides primary medical care to working uninsured adults in Wake County. Comprehensive care at AMM focuses on the body, mind and spirit of our patients and incorporates care and services to help our patients improve health and lifestyle outcomes. AMM provides access to fresh food through our community garden, health education and disease management classes, yoga and other exercise programs, and cooking and gardening classes. AMM patients also have access to mental health services through our pastoral care and counseling program.

Uninsured individuals are at higher risk of going without necessary care even though they are often those with great health needs; about one in six low-income adults are in fair or poor health, and about a third has a diagnosed chronic condition. We are a medical home to 2,500 low-income adults. A typical AMM patient lives in a household of three and supports his or her family on $19,907 a year. For each visit to AMM, patients pay a sliding scale fee based on household income, usually about $25. This fee places value on the services provided and makes the patient an active participant in their healthcare. We know that providing medical care and medication is not enough. Key to supporting our patients is to help identify and address barriers to health. A deep connection exists between physical heath and mental health, and between social issues and health outcomes. We treat AMM patients with that in mind.

Position Duties Include :
*Function effectively in care team with Medical Assistant to provide compassionate care for patients.
*Obtain health history and perform physical exams.
*Arrange for standard of care preventive measures, including cancer screenings and immunizations as needed.
*Diagnose and develop treatment plans with the patients.
*Prescribe medications as allowed by federal, state and professional guidelines utilizing low-cost programs including Pharmacy Assistance Program, MediAssist, and FIGS.
*Arrange referral for necessary labs, imaging and specialty consults utilizing Charity Care programs and Project Access (a Wake County Medical Society program).

Qualifications:
*Maintain current and unrestricted North Carolina medical license.
*Board Certification in Internal Medicine or Family Medicine.
*Skills in managing complex patients .
*Skills in managing medical staff .
*Proficiency in Microsoft Office and EHR experience.
*Spanish speaking a plus.
