Job Description

Community Health Network is seeking a full-time Family Medicine Physician to provide family medicine, chronic disease management, pediatrics, women's health including prenatal care, medication-assisted treatment, treatment for anxiety and depression, with a willingness to lead in this area. The ideal candidate will assist with the development of staff and possess a strong desire to deliver outstanding care and education to patients of all ages. This position will be housed at our Freeport office, where your expertise will be embedded in an integrated care clinic that follows patient-centered medical home (PCMH) guidelines and principles.
As a federally qualified health center (FQHC), Community Health Network provides dental, medical, behavioral health, and pharmacy services within the Freeport, Clute, Angleton, Alvin, Pearland, League City, Galveston, Houston, and Bacliff areas.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Work closely with the Medical Director and other appropriate staff to identify, develop, and implement comprehensive clinical services and preventive services.
Assist the Chief Medical Officer in the evaluation and supervision of clinical staff by collaborating with Nurse Practitioners, Physician Assistants, Nursing assistance, and the clinical team.
Participate in team and evidence-based approaches for health care by providing appropriate patient and family education about illness needed treatment, medications, immunizations, care coordination, population management, and development and implementation of a written care plan.
Participate in quality assurance and performance improvement activities to work toward the CHN clinical measure program goals to achieve quality health care.
REQURIEMENTS/QUALIFICATIONS:
Must be Board Certified and possess a current and unrestricted license by the Texas Medical Board as a Family Medicine Physician
Must be eligible for Medicare and Medicaid provider participation at the time of hire and maintain eligibility while employed with CHN
At the time of hire, qualify for or possess an active and unrestricted DEA
Possess a current PALS certification
Minimum of 2 years of experience in family medicine including pediatrics, gynecology, obstetrics, and medication-assisted treatment, required
Five years of clinical experience preferred.
Experience working in a high-volume clinic
Enjoy working in a multi-cultural environment
Knowledge of professional and ethical standards of health care delivery
Ability to work with people of all socioeconomic levels.
Knowledge of public health principles and practices preferred
FTCA coverage or private professional malpractice insurance obtainable.
Ability to respond effectively to the most sensitive inquiries or complaints.
Ability to read, analyze, and interpret complex documents.

GENERAL INFORMATION
WORKING CONDITIONS:
The position is physically comfortable, the individual has discretion about walking, standing, etc. There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature, and air conditions.
This position is subject to drug testing, professional reference verification, a physical examination, and background screening

Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in similar positions.
ADDITIONAL JOB-RELATED INFORMATION:
Four-day work schedule, 8:00am 6:00pm, 36 hours of direct patient care and 4 hours of direct patient care
