Job Description
International Community Health Services is deeply rooted in the Asian Pacific Islander community that provides culturally and linguistically appropriate health and wellness services and promotes health equity for all. PACE (Program of All-Inclusive Care for the Elderly) is a Medicare and Medicaid program that provides services to keep nursing-home eligible individuals living independently at home or in the community as long as possible, instead of going to a nursing home or care facility. PACE is a part of ICHSs Healthy Aging and Wellness Program (HAWP), which focuses on providing care to the elderly in our community. As a PACE organization, ICHS eldercare experts conduct assessments and design a comprehensive personalized care plan, then provide and coordinate services to address clients health and personal care needs. PACE participants are provided with a number of services including primary care, in-home and nutritional services, and because PACE serves patients from a wide span of the Greater Seattle/King County area, transportation is provided to and from the Legacy House facility.
Under the supervision of the Chief Medical Officer, the PACE Medical Director provides general medical direction and supervision of the medical aspects of care for the Healthy Aging and Wellness Program (HAWP). The PACE Medical Director is responsible for the delivery and oversight of participant care, clinical outcomes, implementation and oversight of the quality assessment and performance improvement program, development and maintenance of medical policies and procedures/standards and protocols. The PACE Medical Director provides direction and supervision to primary care physicians and Advanced Practice Clinicians and leadership of the program among staff, colleagues and providers in the community. This individual is also responsible for providing medical consultation for clinical issues for other services within HAWP, including Assisted Living and Adult Day Services. The PACE Medical Director is responsible for identifying opportunities to enhance the clinical and health care delivery for the Healthy Aging and Wellness Program.
Position Requirements:
Education
Graduate of an accredited medical school and completed a US residency accredited program
Experience
Minimum of two years working with the frail or elderly population
Other Requirements
Valid WA State Drivers License, proof of insurance
WA State Physician License and DEA Registration.
Board certified Internal Medicine or Family Practice required, Geriatrics preferred
Current BCLS
