Care Coordinator - Opioid Use Disorder Treatment

ReVIDA Recovery is seeking Care Coordinators to join our team providing clinical services and care coordination to individuals with opioid use disorder. The Care Coordinator is a critical member of the larger multidisciplinary clinical team.

The Care Coordinator will be responsible for assisting patients with goal-oriented and individualized supports focused on improved self-sufficiency and independence through assessment, planning, linkage, advocacy, coordination and monitoring activities. The Care Coordinator may provide occasional supportive counseling and crisis intervention services when allowed by regulatory or funding authorities within the staffs legal scope of practice.

Virginia RN license required
At least one year of experience working with individuals with substance use disorder either as a primary diagnosis or as a co-occurring diagnosis with mental illness preferred
Knowledge and support of Harm Reduction as an evidence-based practice critical
