Summary
The Social Worker (HUDVASH Case Manager), a professional social worker, in this position functions as the Medical Center's Case Manager for VA Sierra Nevada Health Care System for Homeless Veterans Program's, HUD/VASH Program. The HUD/VASH program is a collaborative effort, supported through HUD Section 8 rental assistance vouchers and VA's provision of intensive case management services.

Responsibilities
This position may be eligible for student loan repayment through the VA Education Debt Reduction Program (EDRP).

The primary goal of HUD/VASH is to move veterans and their families out of chronic homelessness. The primary component of the program is VA case management services, designed to improve veteran's health and mental health, enhancing their ability to remain stable, housed and community-integrated.
The incumbent is responsible for program oversight, monitoring and administration, serving as the primary contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations.

Program Administration and Organization - serves as the medical center's primary point of contact for the National HUD/VASH Program, Office of Mental Health Services, and VACO.
Outreach - regularly conducts and participants in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers or outpatient clinics.
Clinical Assessment - evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and prioritization for admission.
Referral, Rehabilitation, and Program Admission - provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs.
Comprehensive Case Management - develops psychosocial treatment plans, actively involving the veteran and their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment.
Direct Services - provides direct mental health and substance abuse services for veterans and family members or significant others in support of the veteran's treatment.
Evaluation, Education, Supervision, Coaching and Mentoring - establishes a continuing relationship with the veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate.

Work Schedule: Monday through Friday, 8:00am to 4:30pm
