Nurse Manager RN
Pine Haven Community, Inc.
The Clinical Nurse Manager is responsible for the coordination, planning, implementing, and evaluating of nursing services provided to each resident on a 24-hour basis. The Clinical Nurse Manager will supervise and provide guidance to the licensed nurses and registered nursing assistants in accordance with current state and federal regulations.
Complete initial care plans by day 21
Update care plans quarterly and with any significant changes
Attend Care Conferences
Directs and oversees the nursing staff to ensure the nursing procedures are followed and reflect the current policies and procedures of the facility.
Oversee treatment schedules to assure that treatments are being administered as ordered.
Conduct audits as directed by DON
Perform direct care related activities as the need arises. (Work as floor nurse)
Participates in the development of policies and procedures pertaining to resident care as directed by DON.
Perform daily rounds of the nursing unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards.
Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of residents total regimen of care.
Participate in the development, maintenance and implementation of the facilities CQI/QA program.
Complete and investigate accident/incident reports and assist with abuse/neglect reporting to specific agencies as needed, including identification and resolution of trends.
Acts as a liaison between provider, resident, family and resident care team to ensure provider orders are accurately carried out and documented using the information from the residents plan of care.
Oversee tardive dyskinesia assessments to ensure proper timing according to state and federal regulations.
Initiate and resolve follow-up and problem charting as needed.
Ensures completion of admission assessments.
Assists with discharge planning with interdisciplinary team.
Participation in the ongoing review of the written plan of care for each resident for appropriate resident goals, problems, approaches and revisions based on nursing needs.
Guides nursing care including focus on residents changes in condition and implementation of appropriate nursing interventions.
Attends rounds with medical providers.
Serve on, participate in and attend various committees and meetings of the facility as delegated by the DON.
Assists with the coordination of staffing patterns and productive nursing hours as delegated by the Director of Nursing.
On-Call as delegated by the Director of Nursing Services.
Participates in identifying ongoing educational and training needs and provides formal and informal training.
Other duties as assigned/delegated.
Current license in the State of Minnesota as a Registered Nurse
Minimum of 2 years experience in long term care preferred.
