Summary
The Licensed Practical Nurse (LPN) collects patient /family information that assists the Registered Nurse in assessment, planning, implementation, and evaluation. The LPN provides safe, effective care for assigned patients with the assistance and under the supervision of a Registered Nurse.
Description
PRIMARY DUTIES AND RESPONSIBILITIES
1. Patient/Family Data Collection
Performs patient/family data collection to facilitate the identification of clinical, educational, psychosocial, and cultural needs per ACH policy and procedure.
Communicates pertinent patient/family data and observation information to appropriate health team members.
2. Planning
Contributes to patient plan of care based on clinical, educational, psychosocial and cultural needs.
Plans prioritizes, and organizes patient care tasks to ensure time and resource utilization.
Recognizes the need to resource other health care team members to plan patient care needs.
3. Implementation
Completes routine patient care tasks and medication administration in a safe, efficient and timely manner.
Individualizes nursing interventions to meet identified patient/family clinical, educational, psychosocial and cultural needs.
Recognizes emergency situations and intervenes appropriately.
Demonstrates the ability to learn, perform and maintain competencies needed for assigned unit.
4. Evaluation
Modifies nursing interventions to meet changing patient needs in collaboration with the Registered Nurse.
Documents patient care according to department policy.
Modifies and reassesses patient plan of care in collaboration with the Registered Nurse.
5. Professional Performance
Practices cost effective use of supplies.
Utilizes available communication tools (e-mail, staff meetings, and bulletin boards) to remain informed.
Delegates' appropriate tasks to non licensed personnel and ensures completion.
Completes 8 contact hours per year of appropriate continuing education related to current practice area and role responsibilities.
Consistently demonstrates that new knowledge and skills gained from education are incorporated into actual clinical practice.
Uses mentoring and role modeling skills to promote positive patient outcomes and professionalism within the Nursing Division.
Shares knowledge and expertise with nursing staff and other members of the health care team.
EDUCATION REQUIREMENTS
Required - Certificate from college program or technical school (Graduation from an accredited school of Licensed Practical Nursing.)
EXPERIENCE REQUIREMENTS
Preferred - entry level or experienced LPN experience preferred
CERTIFICATION / LICENSE / REGISTRY REQUIREMENTS
Required - Licensed Practical Nurse (LPN): (AR) (Candidate must possesss at time of hire)
SKILL REQUIREMENTS
Required - Mental/Reasoning Requirements:- reading - complex- writing - complex- complex calculations.
