GENERAL SUMMARY:
The ACNEye Carelead is responsible for developingastrategic and operationalvision applicable totheir specialty of training, in close partnership with ACNleadershipand the DHS Director of Eye Care. The ACNEye Carelead is responsible for carrying outthis shared visionby employingleadership andpractice transformationprinciplesin a variety of settingsacross theclinic network.The ACNEye Carelead willdevelopprocesses, expected practices, and policies in the areas of patient access, dataanalytics,clinical care delivery,quality improvement, and utilization management.The ACNEye Careleadwillalso provideexcellentdirect clinicalcare inOphthalmology,whilemodeling appropriateteam-based care and communicationto other providersand ancillary employees.
JOB DUTIES/ RESPONSIBILITIES:
Patient Care Delivery:80%
-Provides directdiagnostic and therapeuticclinical care to patientsin Ophthalmology, which may include outpatient visits, procedures, and surgeries as indicated.May provide clinical services at one or more ACNfacilities dependingon the specific needs of the system.
-Completesall necessarypatient-carerelatedactivities such as communication with patients and/orancillarystaff to achieve treatment goals.
-Engages in necessaryoral,written, and electroniccommunication and completes required patient care EHR documentation/formsper DHS and ACN policies.
-Coordinatescare transitionsbetween ACN andDHSEyeClinics,managesurgent care referrals from ACN and DHS facilitiesas needed.
-Reviews all assignedEyee-consult cases, responds to requesting providers in a timely manner and in accordance with DHS Expected Practices.
-Providesclinical advice and support for ACNEye Careand Primary Careproviders. Models outstanding clinical andteam-basedcare to all ACNemployees.
-Performs otherpatient care relatedduties as assigned by the supervisor, which are in compliance withDHS and ACNpolicies and procedures.
Administrative/Leadership Duties:20%
-Works directly withDHSDirectorof Eye Care,ACNand facility leadership toprovideoversight andrecommendations for ACNspecialtypatient access,clinical care delivery, operations, andqualityinitiatives.
-Worksdirectly withDHSDirector of Eye Care, ACN and facility leadership todevelop, evaluate, and revise ACN or DHSEye Carepoliciesand procedures.
-Performs periodic comprehensive need assessments inOphthalmology/Optometryand addresses each need as indicated in partnership with ACN and DHS leadership/Director of Eye Care.
-DevelopsEye Caredata dashboards in collaboration with CMIO and informatics team. Implements a data informed approach to guide clinical and quality operations.
-Develops a utilization management framework to assess appropriateocularclinical care delivery across ACN. Performs comprehensive clinical quality audits as indicated. Uses robust constructive feedback to provide useful information to specialty providers. Partners with select providersand supervisorsto guide performance improvement as necessary.
-Provides guidance and develops processes around practice transformation initiatives, such as virtual healthcare, preventative care,outcomes-basedprinciples, health equity interventions,etc
-Provides provider, nursing and ancillary staff education and training as necessary to support clinical operations.
-Activelyparticipatesinassigned DHS or ACNcommittees/workgroupsas necessary.
-Effectively represents the unique interests and perspective of the ACN at DHS committees/workgroups.
-Reports relevant activities andupdatesto ACN and facility leadership.
-Chairs the relevant ACNSpecialtyQuality Board committee. Develops QI best practices in collaboration with the committee members. Reports to and takes direction from the ACN Quality Board committee.
-Chairs other ACNSpecialtycommittees as directed by ACN leadership.

High Desert Regional Health Center (HDRHC) is located in Lancaster, California. It is a new facility that serves the Antelope Valley community, which includes the cities of Palmdale and Lancaster.
