Duties

Summary
The VA Texas Valley Coastal Bend Health Care System is currently seeking a Physical, Medicine & Rehabilitation Physician who will be responsible for patient care, patient/family education, and participation in indicated clinical research in PM&R.

Responsibilities
The Physician at VCB provides comprehensive care for patients with illnesses within the scope of ambulatory PM&R care. The majority of the Physician's time is devoted to patient care and education, including maintaining proficiency in the broad spectrum of ambulatory PM&R care. Remaining time is set aside for performance improvement activities and potential research to improve patient care.

The Candidate should possess leadership skills in coordination of activities of various ancillary staff, house staff, therapists, other professionals, and students from various disciplines. He/she should have experience in diagnostically and therapeutically managing patients who present with the broad spectrum of conditions addressed by PM&R in outpatient setting-including but not limited to care of spinal cord injuries, amputee, Musculoskeletal conditions, basic neuromuscular and neurocognitive pathology, perform secondary TBI screen, review for appropriateness of and recommend mobility assistance devices, home adaptation requests to meet needs of medical condition, and work with other providers/specialists to coordinate care and review appropriateness of physical therapy ordered and rendered. Furthermore, the Physician should demonstrate knowledge of preventive medicine and primary care as related to PM&R (e.g. complications of disorders & treatments, side effects of medications used), as well as the role of non-physician health care professionals.

Additional duties include the following:

Administrative
- May be required to perform administrative duties required for performance measures or improvement activities, compliance with accrediting organizations, or other duties as assigned.
- May be required to serve as preceptor for health care students rotating through the outpatient clinics.
- May be required to lecture or make presentations on Physical Medicine & Rehabilitation to clinic staff.
- May be required to participate in emergency preparedness activities as a result of national health disasters.
- May have to participate in various committees involving wound care, skin integrity, pain, non-invasive procedures
- Respond promptly to any requested information for such as Medical Advisory Opinions, congressional inquiries, peer reviews from other facilities

Clinical
- May be required to serve as preceptor for health care students rotating through the outpatient clinics
- May be required to lecture or make presentations on Physical Medicine & Rehabilitation to clinic staff
- May be required to participate in clinical research.
- May be required to participate in emergency preparedness activities as a result of national health disasters
- May have to participate in various committees involving wound care, skin integrity, pain, non-invasive procedures
- Respond promptly to any requested information for such as Medical Advisory Opinions, congressional inquiries, peer reviews from other facilities.
- Orders diagnostic studies such as laboratory tests, X-rays, electromyography/Nerve conduction studies(EMG/NCV), electrocardiograms (EKGs) and other tests when clinically indicated.
- Consults other disciplines, as necessary, to complete the patient examination; e.g. various med/surg specialties, Occupational Therapy, Physical Therapy, neuropsychological testing, audiology and speech/language pathology testing, vision testing, dental evaluation, psychosocial evaluation, etc.
- Actively direct amputee/spinal cord/and wheelchair clinics within scope of practice and resources in ambulatory setting
- Carries out health promotion and disease prevention activities including education and shared decision-making
- Provides education and counseling to patients and families in preventive care, medical conditions, and the use of prescribed treatments and drugs.
- May be required to provide clinical oversight, consultation and/or patient care management assistance to the PA/NP assigned

Work Schedule: 8:00 a.m. - 4:30 p.m. (Monday through Friday)

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR
[(2) Those approved by the American Osteopathic Association (AOA),OR
(3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences.
Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs.
Proficiency in spoken and written English.
Preferred Experience:
Board Certified/Board Eligible in PM&R
Outstanding clinical & refined interpersonal skills
2+ years clinical experience
Preferred NCS/EMG and botulinum toxin procedure experience
Experience in the diagnosis and treatment of general outpatient PM&R related diagnoses including spinal cord injury, limb loss, stroke and brain injury

Physical Requirements: Heavy lifting, 45 pounds and over; moderate carrying 15-44 pounds straight pulling (1 hour); pushing (1 hour); reaching above shoulder; use of fingers; both hands required; walking (3 hours); standing (3 hours); kneeling (1 hour); repeated bending (1 hour); Ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; and hearing (aid permitted). Education
Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
