Education:
Requires M.D., or professional equivalent degree
Must have successfully completed or are in the process of completing an accredited Residency Program
Certifications, Licenses and Registrations:
Must be/be able to become a licensed Physician / Surgeon in the State of Illinois
Must also maintain an IL Controlled Substance license and Federal DEA license
Must be board certified or board eligible in Pediatrics
