Job Description:
Engage in the full-time practice of family medicine, plus call coverage. Order, perform, and interpret tests and analyze records, reports, and examination information to diagnose patients conditions. Monitor patients conditions, medical progress, and reevaluate treatments as necessary. Perform annual preventative medicine examinations, evaluate various health metrics, complete procedural inventions, and consult with patients to mitigate health risk factors. Conduct hospital and long term care facility rounding on assigned patient population. Refer patients to specialists when needed for further diagnosis or treatment. Prescribe medication as necessary.


Job Requirements:
1. Doctor of Medicine degree (MD) or its foreign equivalent.
2. Current and valid Physician License from the state of Indiana.
3. Completion of a three (3) year residency program in Family Medicine.
4. Board certified or Board eligible in Family Medicine.
5. Weekly travel required to local assisted living facilities.
6. Must have current authorization to be employed in the U.S. without employer sponsorship.
