RN Outpatient Coordinator- Care Management

Overview:
The RN Outpatient Coordinator works under the supervision of the Care Management Leadership and is responsible for the coordination of care to patients who are considered high risk with complex medical and psycho-social needs. The Outpatient Coordinator strives to promote coordinated care in order to improve outcomes efficiency in utilization of health care services by performing the following core functions: a) Assessment: Identification of patients for case management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care b) Planning: Collaboration with the patient family/caregiver primary provider community partners and other members of the health care team for developing an effective plan of care c) Facilitation: Care coordination and communication among all involved partiesd) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate timely care is received. The position requires a high degree of flexibility independence and willingness to participate in multiple activities with local travel. The Outpatient Coordinator must have strong communication skills and the ability to converse comfortably with patients caregivers physicians community partners and the interdisciplinary team.

Riverside began serving the Virginia Peninsula community in 1916. In 1963, Riverside moved to the present 72-acre location in Newport News on J. Clyde Morris Boulevard. A 450-bed facility, Riverside Regional Medical Center is the Virginia Peninsulas most comprehensive medical facility combining the most recent computerized medical technology with a healing environment. The Pavilion is a 250,000 sq. ft., $107 million, 3-story addition to Riverside Regional Medical Center, which opened in 2013. This acute care facility provides a regional cancer care center, the Peninsula regions only comprehensive stroke center, the areas only open-heart surgery program, the regions only Level II Trauma Center, and a cornerstone neurosciences program that includes the Chesapeake, Riverside and University of Virginia Radiosurgery Center.

Qualifications
Education: ADN or Diploma in Nursing required; Bachelor's preferred or agrees to obtain his or her Bachelor of Science in Nursing (BSN) degree
no later than 5 years (60 months) from date of hire.


Experience: 5 years RN experience in acute care setting Required: Prior Care Management Experience Preferred and Prior Emergency Department Experinece preferred.


Health insurance and medical management experience
Knowledge of current Medicare and Medicaid regulations
Previous experience in Home Health, Utilization Management, Discharge Planning
Disease Management or Health Coaching

Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health system and facility goals and strategic plans.
