Annashae Custom Staffing is currently seeking MOD Physicians on behalf of three facilities in Illinois. RESPONSIBILITIES: • Initial history and physical examination • Evaluation for admission • Initial diagnosis • Devise initial treatment plan and make recommendations for treatment • Sign admission certificates • Perform admission history and physicals • Order lab studies • Provide follow-up evaluations and care to recipients • Document exam results and treatment plans • Attend to employee work-related injuries prior to hospital referral • Respond to resident emergencies (whether medical or psychiatric) • Consult with attending physicians as necessary • Make referrals to acute hospital • Sign and review restraint or seclusion orders • Provide on-call services QUALIFICATIONS: • Current Illinois license in Primary Care, Rehabilitation Medicine or Psychiatry • Board certified in Primary Care, Rehabilitation Medicine or Psychiatry • Current Illinois Controlled Substance certificate • Current DEA certificate • Mantoux skin test prior to starting work or documentation of negative Mantoux within last 90 days • Experience working with psychiatric patients • Malpractice is state-indemnified SCHEDULE: • Weekends: 5:00pm — 8:00am (63 hours) — Friday evening to Monday Morning • Weekdays: 5:00pm — 8:00am (15 hours) PAY RATE: • Competitive Wages • Travel/lodging reimbursement • Malpractice coverage • Willing to reimburse renewal or application for IL license JOB TYPE: • Contract REQUIRED EXPERIENCE: • Work with psychiatric patients: 1 year REQUIRED LICENSE AND CERTIFICATIONS: • Current Primary Care, Rehabilitation Medicine or Psychiatry in Illinois • Active Illinois License in Primary Care, Rehabilitation Medicine or Psychiatry • Board Certified in Primary Care, Rehabilitation Medicine or Psychiatry • Current DEA Certificate
