Who We Need We’re looking for someone with the following skills, experience and credentials: •Must be at least 21 years old. •Direct service providers who want to provide and be of service, by going to rehab and house hunting activities. •Establish positive, effective, and professional relationships with person served, families, colleagues and other components of the AHCCCS, Maricopa County Regional Behavioral Health Authority and other stakeholders. •Conducts assessments, determines eligibility, and develops individualized case plans. •Provides case management services in conjunction with community partners. •Document and maintain up-to-date information on services provided to individuals Medical Record. •Monitors and maintains on-call system. We have full-time Integrated Health Case Manager that works Monday-Friday, 8AM-5:00PM. Starting pay is $15 per hour. Locations in Phoenix, Glendale, Peoria, Gilbert and Mesa. Requirements To be considered a qualified behavioral health technician, a person must have one of the following combinations of education, license and/or behavioral health work experience: •Master’s degree in a behavioral health related field. •Bachelor’s degree in behavioral health related field required. •Master’s degree in non-behavioral health related field and 30 semester hours in behavioral health education. •Bachelor’s degree in non-behavioral health related field and at least (1) year of full-time behavioral health work experience. •Associate’s degree in behavioral health related field and two years of full-time behavioral health work experience. •Associate’s degree in non-behavioral health related field and 30 semester hours in behavioral health education and three years behavioral health work experience. •High school diploma or high school equivalency diploma and four years of full-time behavioral health work experience •Ability to obtain and maintain a Level One Fingerprint Clearance Card. •Valid AZ Driver’s License and MVR clearance is required. These positions do require driving company vehicles to transport members. •Must be able to lift minimum 50 pounds, stoop and bend comfortably in all positions.
