Physical Therapist - Home Health POSITION SUMMARY: Responsible for the evaluation, clinical assessment, treatment plan, performance of therapeutic procedures, and progression of physical therapy patients. Responsibilities also include consultation with medical staff and health professionals, completion of clinical records and insurance forms, and supervision of assistive personnel.
