The Addictions Clinical Case manager is directly accountable to the Clinical Psychotherapist, Addictions Specialty. The candidate provides clinical case management services and supportive counseling as assigned to Medication Assisted Treatment (MAT) and/or substance use disorder (SUD) clients receiving behavioral health services and COH patients or housing clients who need to be linked to substance use services. The candidate outreaches and engages community and community-based agencies to link with Medical Assisted Treatment program and/or substance use counseling services. In relation to remote work versus on-site expectations, this position is classified as the following: Required to work on site most days in the work week with some opportunities to work remotely. Please note that the following is subject to change at any time. This is a full-time position. Highlighted Duties and Responsibilities: Performs intake assessment within one week of the receipt of referral and completes additional screening as needed Participates in quality management activities, including peer review which occurs every other month. Meets regularly with patients on case load to develop case plans and appropriate interventions. Monitors client progress through the course of treatment, re-evaluating and adapting the case plan at required intervals Provides supportive counseling and/or groups as needed for Medication Assisted Treatment and/or substance use clients as directed and supported by supervisor Provides case management support to the MAT Program and Behavioral Health SUD clients, which includes but is not limited to identifying and addressing psychosocial concerns, engagement in services, scheduling services, coordinating care, advocacy, and health related barriers. Reviews and monitors CRISP Reports for clients who have had an opioid and/or substance use related hospitalization. Follows up with these clients to ensure they are connected to behavioral health services at COH or with a community mental health provider within seven days and a second follow-up within 30 days Ensures patient safety by following COH policies for contacting Child and Family Services Administration, Adult Protective Services, Mobile Crisis, and/or 911 when appropriate. Creates patient safety contracts as appropriate Engages with the community and internal departments, answering COH MAT program and substance use related questions and telling potential clients about the services offered Identifies and attends at least one community event a month to represent COH MAT program and substance use counseling services by tabling at community and health resource fairs/events Networks with community resources for housing, employment, substance abuse treatment, day care, training, etc., and makes referrals as appropriate Completes and locks all assessments and progress notes within 3 business days after patient visit Secures patient information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.
