This APP will be based out of our Frederick Health Medical Group -- Urbana Primary Care office. Candidate must be willing to float to other office locations as needed to cover vacations and open urgent care shift work, as needed. All practices are PCMH accredited and ACO participating, with very strong supporting structure that includes care management, behavioral health, pharmacy, etc. There is a strong culture of provider leadership within FHMG. The position is open to a Physician Assistant or Nurse Practitioner. The APP will support the Mission of Frederick Health System and complies with the Code of Conduct. The ideal candidate will: •Possess certificate of completion from an approved program for Adult Nurse Practitioner or Certified Physician Assistant •Be eligible for licensure by the Maryland State Board of Nursing or the Maryland Board of Physicians. •Have sufficient medical training to qualify legally and professionally to perform all demands placed upon them.
