Job Summary UCNW believes mental & emotional health is an important part of overall health and wellness. We aim to be a medical home to our patients, providing the services necessary for a healthy mind & body within a single health center through our Primary Care Behavioral Health (PCBH) program. In this program, behavioral health services are integrated with medical care to help patients meet their health care goals & access other community resources needed to be healthy such as housing, food & legal assistance. As the Primary Care Behavioral Health Supervisor, you would oversee the PCBH program at UCNW’s medical clinic. This would include managing, evaluating, & coordinating the program while providing supervision, including clinical supervision, to the PCBH team of clinical social workers, counselors & psychologists. Responsibilities also include ensuring the financial sustainability of the program & providing direct services to patients in the Behavioral Health Consultant (BHC) role. The BH Consultant (BHC) is a unique role that bridges the gap between traditional mental health care and Primary Medical Care. The BHC works within the primary care clinic’s health care team and collaborates with the primary care provider (PCP) in managing the overall health of their enrolled population. The BHC also works with specialty behavioral care, including counselors and psychiatric staff, by managing behavioral needs at the lowest level of care, coordinating between internal levels of care, and supporting patients as they exit specialty care. As the BH Consultant, you would help improve recognition, treatment, and management of psychosocial/behavioral problems and conditions presented by patients. You would provide consultation services to patients referred by the primary care team & deliver brief, solution-focused services to patients and PCPs using the integrated care model. You would also respond to urgent and emergent BH issues that arise in the clinic & work with patients to identify health goals and to achieve behavior change to help achieve those goals. You would additionally identify and reduce challenges in life domains which impede the achievement of health goals.
