Job Title: Family Practice Physician (MD, DO) Full-Time / Benefited Hours: Monday – Friday, 8:00am to 5:00pm Worksite: Santa Rosa Department: Medical Department FLSA Status: Exempt Reports to: Medical Director Salary Range: Application Closing Date: $211,494.00 to $349,419.00 per annum Open Until Filled Why Work for Us? Job Purpose: The Family Practice Provider will work in collaboration of his/her assigned care team in providing quality primary care services to patients of all ages in accordance with accepted standards of medical practice. Responding to medical/health problems presented by patients including history taking, diagnosis, investigation, treatment, and referrals as appropriate; maintaining confidentiality and impartiality; promoting health education in conjunction with other health professionals; inclusive of taking night and weekend telephone calls shared with other staff clinicians, keeping up to date with medical developments, new drugs, treatments and medications. This involves the delivery of family-oriented medical care services within a Patient-Centered Medical Home Model. Qualifications: • Graduate of an accredited school of medicine or osteopathy • Current unrestricted license to practice in the state of California. • Licensure and certification according to provider status: MD, DO • 2+ years of experience is preferred. • Possession of a Valid Drug Enforcement Administration Certificate. • Board Certification in Family Medicine or other closely related Primary Care specialty (preferred) • Ability to cover rotating nights & weekend telephone calls. • Obstetrical consultation readily available • Must possess current CPR certificate. • Must show proof of completed COVID 19 vaccines, booster, and annual flu vaccine.
