About the Organization
CFH is a leading provider in the NYC homeless healthcare and housing sector, serving 9,000+ homeless individuals annually at 27 federal qualified health centers across the city and 400-500 individuals at three transitional shelters, all of which have on-site health services. CFH is distinguished from other organizations in the homeless healthcare sector by 1) being NYCs largest service provider with a citywide network of 27 health center locations serving homeless and unstably housed people, which we are expanding in the years ahead; and 2) providing the same high-quality health care to homeless individuals as those who are not experiencing homelessness receive. The quality of our services is verified by the National Committee on Quality Assurance (NCQA), which has designated 14 of our health centers as a Patient-Centered Medical Home. to care.
About the Community
Over 88% of our patients are 100% and below the federal poverty level. Our patient population is disproportionately comprised of persons of color, with 85% identifying as a person of color, largely a result of decades of systemic racism that has prevented the acquisition of financial and tangible assets by communities of color such as housing/real estate, as well as access to quality health care. CFH serves homeless persons in all 5 boroughs of NYC, providing high-quality health care in communities with the highest needs and lack of access.
