HopeHealth provides a health care home to over 50,000 patients and, through our Hope Fund, we are able to provide specialty care and support to many of those patients with complicated financial needs and health conditions.
As one of 23 Federally Qualified Health Centers in South Carolina and more than 1,200 nationwide, HopeHealth is able to offer reduced fees to the uninsured through a sliding fee scale. HopeHealth services include primary health care for all patients ages and a variety of special services.
HopeHealth educates our patients on the importance of having a health care home. As a patient-centered medical home, HopeHealths medical team works to prevent and detect illness and the early onset of disease, provide routine physical examinations, and promote overall healthy lifestyles. Our health care providers offer access to basic health care services and integrate a range of health resources to provide the very best in patient care, prevention, and support services.
At HopeHealth, we go the extra mile to treat the whole person. Instead of focusing on a specific symptom, our providers and staff assist patients with determining the underlying causes of poor health. Sometimes this is as simple as coordinating transportation to appointments or assisting in making referrals to other social service providers. Other times, its a quick referral to a HopeHealth specialty service provider. Our integrated approach to health care truly places the patient at the center of care.
About the Community
HopeHealth provides integrated health services for more than 50,000 patients in areas surrounding Aiken, Clarendon, Florence, Orangeburg, and Williamsburg counties. It was founded in 1991 as a grassroots HIV/AIDS support organization, and was designated as a Federally Qualified Health Center in 2007 to expand its services to include primary and pediatric care. HopeHealth now has 18 locations (16 community health centers and two infectious diseases treatment and prevention centers) and over 120 providers covering primary care, dental care, chiropractic care, rheumatology, endocrinology, behavioral health and psychiatry, pain management, substance use treatment, and womens health services.
