About the Organization
TriValley Primary Care is the premier independent primary care group practice in Bucks and Montgomery Counties, Pennsylvania. We have eight community-based practice locations and a geriatrics division dedicated to providing on-site care to several local continuous care retirement communities. TriValley serves as an anchor practice in the Accountable Care Organization, Tandigm Value Partners, which has proven successful in the Medicare Shared Savings Program. TriValley prides itself on a culture which puts the patient first and is dedicated to achieving healthy outcomes.
About the Community
TriValley Primary Care is an independent group practice owned by the physicians that practice within our offices. With operations in Bucks and Montgomery Counties in Southeastern, Pennsylvania, we enjoy a balanced mix of suburban and rural geographies. Most of our offices are located roughly 45 minutes outside of Philadelphia, with some locations much closer. TriValley partners well with our local hospitals and specialist communities.
